Patient Information


Insurance

We bill most insurance carriers for covered services, as long as proof of insurance is provided at the time of service. Co-payments are expected at the time of service. Payment for services is the responsibility of the patient or guardian of the patient. We accept cash, checks, Visa and MasterCard.

We participate with the following insurance companies: Aetna, Anthem, Medicare, Southern Health, United HealthCare, Cigna. (disclaimer: Although this list is accurate at the time of posting, our participation status may have changed. Please confirm our status when making your appointment.)

Noncovered and Cosmetic Services

Payment is expected in full at the time of service for any non-covered or cosmetic procedure.

Missed Appointments

Our call center will notify you with a reminder call at least 48 hours prior to your scheduled appointment.

We ask that 24 hours notice be given to cancel an appointment. There will be a $20.00 charge for insufficient cancellation.

Telephone Calls

Please feel free to contact our office during normal business hours if you have questions regarding your condition, medication or treatment. If it is necessary to speak with a physician, we will return your call as soon as possible. Because of the visual nature of dermatologic diseases, it is usually not possible to diagnose and treat new problems over the phone. Additionally, changes in your condition may also necessitate your being seen by a physician.

Prescription Refills

Prescription refills are handled by our staff during normal business hours (Monday - Friday, 8:30 a.m. - 4:45 p.m.). Prescriptions will be called into your pharmacy within 24 hours.

Patient Forms

We look forward to a long-lasting relationship with you. You can print and complete the necessary forms and bring them with you to our office where your appointment is scheduled. Please arrive 15 minutes early to complete the registration process. Thank you!
